This is a Bimba BF-091-D Air Cylinder, (1-1/16 bore x 1" stroke) mounted onto a bearing block. The block contains bearings and guide rods. There is also a bracket mounted to this block that has mounted to it a Mead MV-15 Roller Valve. There are also 4 SMC push/pull air fittings for 1/4" tubing. This unit is in good condition and appears to have been used very little.
